Is brushing and flossing alone the best way to prevent periodontal disease? Maybe not.

For years, dentists have advised their patients that brushing and flossing are cornerstones of effective oral hygiene. But is that enough? That may not be the case, according to a new article in Digital Dentistry & Healthcare Knowledge (DDHKMAGAZINE.COM VOL. 4 ISSUE 01 SUMMER 2024). This publication for dentists examines the causes of periodontal and peri-implant disease and explores measures to prevent implant loss.

Titled “Peri-Implant Disease: What Your Patient Can Do to Prevent Implant Loss; What You Can Do to Prevent Patient Loss,” it casts doubt on the effectiveness of brushing and flossing alone in reducing plaque. “Periodontal and peri-implant disease occurs daily,” says Herb Moskowitz, DDS, chairman of ioTech International. He emphasizes, “The best defense against these diseases is a daily routine that includes brushing, flossing, and antibacterial rinses.”

In a six-month study of over 300 dental patients, brushing and rinsing was found to be 18 times more effective than brushing and flossing. Another second study showed that brushing and rinsing was six times more effective at reducing plaque. In addition, a study of 160 dental patients showed that brushing and rinsing was far more effective at reducing plaque and inflammation, as measured by the plaque index, modified gingival index, and bleeding index. Moskowitz stresses the importance of incorporating rinsing into daily oral hygiene. Rinsing your gums at home with a water flosser or oral irrigator can significantly improve oral health. The article points out that using a rinse alone can reach 21% of a 6mm pocket, but when combined with a water flosser, you can reach 90% of the same pocket.

“Now that it is clear that rinsing and subgingival irrigation are critical to successful prevention of periodontal and peri-implant disease, it is equally important to use the most effective mouthwash or mouth rinse. Not all mouthwashes are the same,” says Moskowitz. “Molecular iodine has been shown to be far more effective than any other professional rinse in keeping gums healthy.”

The Digital Dentistry & Healthcare Knowledge article compares different antiseptic rinses against key bacterial pathogens – Fusobacterium nucleatum and Prevotella. Both pathogens are primarily responsible for periodontitis, and Fusobacterium is also directly linked to colon cancer. The results clearly documented “a tremendous difference in efficacy between the antiseptic rinses tested, ranging from complete inactivation of both pathogenic bacteria by the molecular iodine rinse to no biocidal effect whatsoever by a stabilized chlorine dioxide rinse against Prevotella intermedia. Molecular iodine has been shown to be a highly effective agent for the prevention of peri-implant disease. Unlike conventional antiseptics, molecular iodine provides broad-spectrum antimicrobial activity without the development of resistance. Its powerful antimicrobial properties are effective against bacteria, viruses and fungi commonly associated with peri-implant disease. Molecular iodine destroys the cell walls of these pathogens, resulting in rapid microbial death and reducing biofilm formation on implant surfaces. In addition, its use as a rinse or irrigant is well tolerated by patients and does not cause irritation or staining compared to other antiseptics.
